Why do physicians need to be authorized to do medical fitness examinations?
Only physicians who are specially trained and qualified know what to look for in the medical fitness examination of a seafarer. Seafarers on ships operating world-wide often travel for months through the world’s different climate zones. On longer ocean voyages shore-based medical care might not be available for several days due to the limited operating range of shore-based rescue helicopters and lifeboats. Ocean-shipping is in this respect only comparable to space travel. Therefore the Medical Maritime Service of the BG Verkehr allows only selected and highly qualified physicians to conduct the German medical fitness for sea service examinations. The Maritime Medical Service regularly trains the recognized medical practitioners and verifies their professional activity on site. The BG Verkehr has recognized medical practitioners in Germany and abroad.

Qualifications of the recognized medical practitioners
Physicians have to fulfill, inter alia, the following conditions:
- medical specialist for general medicine, occupational medicine, anaesthesiology, surgery or internal medicine,
- at least four years of hospital or ambulantory work experience with a focus on detection and treatment of diseases relating to the activities on board a sea-going vessel,
- at least four weeks of practical experience on a seagoing vessel and knowledge of the health requirements of the service on board a ship, and
- at least 300 medical fitess examinations performed per year.
The conditions of the authorization are set out in §§ 9, 10 of the German "Maritime-Medizin-Verordnung" (Ordinance of maritime medicine).
Comparable international standards do not exist. Every flag state determines by itself when a physician is recognized as qualified. Aside from the qualifications of the physicians, the German medical fitness examinations offer a lot more advantages - find out about them.
